Why this form is requiredHorizon Properties' Uniform Standards of Qualifications require each applicant to complete and sign a Personal Information Release Form authorizing the landlord or landlord's agent to obtain information about credit, criminal, rental and employment history.

What you are authorizing

By signing below, you authorize Horizon Properties LLC, Horizon Properties Rental Group LLC, and their owners, employees, agents, screening providers, and representatives to obtain and verify information reasonably related to your rental application and housing eligibility.

  • Credit and consumer-report information, including information supplied by consumer reporting agencies.
  • Rental history, including payment history, lease history, eviction records, and rental references.
  • Employment and income information from current or former employers and lawful verification sources.
  • Criminal-record information and other public-record information to the extent permitted by applicable law.
  • Information supplied by personal, professional, or other references identified by you.
  • Identity and application information reasonably necessary to verify the information you provide.

This authorization is intended for lawful tenant screening and verification. Horizon Properties may use a consumer report or tenant-screening report in evaluating an application. Applicants retain rights provided by the Fair Credit Reporting Act and other applicable laws.

Protecting sensitive information

Do not enter a Social Security number, driver's-license number, bank-account number, or other highly sensitive identification information into this website form. Horizon Properties may request additional identifying information through a separate, appropriate screening process when needed.

SCREENING RIGHTS

If a rental application is denied, or other adverse action is taken based in whole or in part on information in a consumer report, applicable federal law may require an adverse-action notice identifying the consumer reporting company and explaining certain applicant rights. Applicants may have rights to obtain and dispute information in a tenant screening report.
